How can I make an appointment?

You can schedule an appointment by giving the office a call at (937) 879-1321, and our team will be happy to help you. Alternatively you can send us an e-mail with your personal info / new patient forms and our team can send you back a list of available times for you to visit. 

What do I need to bring with me to my first appointment?

Please bring the following items with you to your appointment:

  • Patient Information Form
  • Dental Insurance Card (if applicable)
  • Identification such as Driver's License, Military ID or State ID 
  • Patient Health History Form
  • HIPAA Consent Form
  • Patient Authorization Form

Do you offer Insurance?

We accept most traditional insurance plans, contact our office to verify acceptance of your plan. Smile Innovations does not participate in Medicare or CareSource. We also do not participate with Health Management Organizations; however, we will be happy to file your insurance claims for you. 

Where is the office located?

Our office is located directly off of I-675. Take a left off of Exit 20 and we are 30 seconds down the road on the left.

Do I need to arrive early for my first appointment?

Yes. Please arrive 10-15 minutes early to fill out any remaining patient forms.

What payment options do you offer?

We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
